Please complete the following information for each individual planning to attend the 2012 CAPI Conference in Las Vegas, Nevada.

 

Upon successful completion of this form, you will be directed to a payment page with options to submit your registration fee. Please note that registration fees DO NOT INCLUDE hotel stay or any meals not listed below. Fees for the 2012 CAPI Conference are as follows:
